  • no pre-exisiting condition limits

this is a discount plan, not a dental insurance plan. exclusions apply:

  • It cannot be used in conjunction with another dental plan, dental insurance, or financing program such as CareCredit.
  • This plan is only honored at Town Square Family Dentistry. It cannot be used at any other dental or specialist office.
  • This plan cannot be used for hospitalization or hospital charges of any kind.

member limitations

  • Any unused services within the patient membership are nontransferable to other plan members regardless of the service.
  • Any missed appointments or cancellations not made within 48 hours of the appointment may be charged a cancellation fee.
  • All payments are due in full at the time services are rendered.
  • No refunds.
